I bought the one like meme said, and it worked okay but it was bulky. The ones made by my dentists are much more compact and they fit better. I actually got a new one today, because I broke the last one.

I use a mouthguard that only goes over my two front teeth. It was originally designed for people who suffered from migraines but has been amazing for me for tooth grinding. I got it from my dentist and it was costly (>$400) but it is extremely comfortable to wear at night and it has fixed my problem of grinding. In addition it has made my headaches and muscle tension in my neck and back disappear. It is amazing how much grinding can effect your entire body, let alone your ability to get a good nights sleep!
Check out the website or ask your dentist about it:

If you talk to your dentist, he/she can make the mouth guard so it fits perfectly in your mouth so it doesn't fall out or so you don't choke on it. Mine I know couldn't choke me b/c of the way it fits in my mouth. Talk to your detist!

I use a normal mouth guard made of a thick sheet of plastic and it was custom made by my dentist after taking a cast of my mouth. I first used one that fitted on both upper and lower teeth but now I wear only the upper one. A custom fitted mouth guard can never fall off or choke you I guess. Very rare it can do so as it fist very tightly.
