Permanent Crown came out for the third time

I have had an experience happen to me. I have had a Permanent Crown come out for the last three times now. I have been to my dentist so far three two times and now on Friday morning, it came out again. It was guaranteed to stay in for good and then this happened. I haven't been eating anything sticky or chewy so I don't why it happened. I have an appt. with my dentist tomorrow afternoon (2:00 p.m.) and I am going to ask him why it came out and then I am going to ask him if he suggests me getting a new one, I am going to ask his office to pay for it, since it is not my fault it came out.
I need advice by 12:00 p.m. tomorrow on what I should tell him etc.
