When to have transplant?

Hi. I’m new to the forum. I have PKD. My creatinine is 4.6. My GFR is 14. When my creatinine level reaches 10 and my GFR reaches about 6, projected to be in less than a year and a half, I will need a transplant. I have a bunch of questions that I’ll write in separate posts.
I’m male, 40 and I weigh 205. For people at similar weights, at what creatinine or GFR have your doctors said you will need a transplant? Or at what numbers did you have one? Thanks.
