I am having a hard time with my Long Term disability application, because I went to a few different doctorss before getting diagnosed. Why is this so hard? It should not be that confusing..but it is. My brain can't get all this straight in order to fill out the forms! I've tried to fill this out off and on since I got the forms two weeks ago...THIS is part of my disability. (brain malfunction)
The problem is that when the symptoms started I went to the doctor, and had to change doctors and didn't get a diagnosis until almost a year later. Here are the doctors A.) Dr. W: - from Dec. 2006 - June 2007 no diagnosis, just treatment of symptoms, and she sent me to a crappy cardiologist too, then on my own I went to a better Cardiologist. B.) Dr. V: - went to her July - Aug 2007. She diagnosed me and treated with some medicine that didn't help me., then gave up on me when it didn't work... C.) Dr. S: I started seeing in Oct. 2007 - present...in which she confirmed diagnosis and has treated me since then. Questions I am having trouble with: 1.) Date first treated by a physician..- A. Dr. W. - the one I went to who did not diagnose me from Dec '06 - June '07 or B. Dr. V., who actually diagnosed me in July '07 2.) First medical attentention for the current disability was given by: How is that different from the above question? 3.) Have you consulted any other physicians or been hospitalized in the last three years? Do they really mean ALL physicians, or just other physicians I have been to for this illness? Do I list my cardiologist, podiatrist, optometrist, OBGYN, Allergist... etc.? Posted on 07/17/08, 10:07 pm |

that is confusing , my wife helped me fill mine out, she's been through it before. they ask the same questions diferently to try to trip you up.
I think the first doc would be the one that first treated you for it without diagnoses n the 2nd question would be the name of the doc that treated you , it's best to read through the whole thing before you fill it out and make at least 5 copys of everything , I remember we used quite a few copys , and if you don't have at least 2 copys of the papers with them not filled out yet make copys in case you mess up. number 3 is mainly for the illness of disability you are applying for, not stuff like you went to hospitol for a few stitches,etc. the more disabilitys you have the better your chance of geting ssi or ssd is what I was told. I got a lawyer for my disability after my first denial, I think it was worth getting a lawyer, but I read on here there was some other organization that will help you , I forgot what it was.,

the first doctor you went to for this condition would be answer for first question. whether or not the doctor could diagnois it or not. the first one you went to with the complaint..
Next list all the doctors from the beginning to current that you went to for this condition. All doctors, specialists, hospitals for all testing. You have to list everything....from when you first went for the condition to now....good luck
